The service is designed to empower whanau to achieve their health goals. We offer a free 'Whanau Wellness Plan' developed by you and your whanau with the assistance of our registered nurse if required. This includes a mini health check; goals and objectives; regular monitoring and review; face to face health education. We cover all aspects of health, but some of the health issues we commonly deal with include smoking, asthma, diabetes, nutrition and physical activity, men's issues, sexual health, advocacy and support, and whanau ora community Kaimahi.

Our community Kaimahi:

  • Ensures that Maori get access to Primary Health Services - GPs, food bank, dental
  • Promotes Kaupapa Maori Health Services to GPs and other health providers in the community
  • Identifies gaps where whanau are not accessing GPs or health providers and reasons why

We provide services to all people, young and old. Men/Tane are especially encouraged to access our service. We can arrange to see you at our clinic in town or our service can also be  delivered at home if appropriate.


The Targeted Youth Health Service (TYHS)

Available in Nelson and Blenheim, this is a primary healthcare service, the purpose of which is to:

  • Improve access to primary healthcare of young people with high needs, specifically students in Alternative Education and Youth Transition Services.

  • Ensure that appropriate and timely referrals are made in order to improve health and reduce inequalities.

  • Promote overall improvement in the provision of services to young people in this district through developing workforce and best practice approaches.

The development of this service will help link students with primary health care providers. The TYHS should also lead to improved access to appropriate services such as mental health, drug and alcohol, nutrition, weight and exercise service, and positive youth development programmes.
The service will aim to improve youth health by improving access, assessments and referrals by giving students in low socio-demographic circumstances the opportunity to access a youth health nurse.

The focus of the service is on improving outcomes for individuals and reducing inequalities, supporting early detection and treatment of problems related to vision, hearing, behaviour and development, and other health and disability conditions to facilitate learning, school performance, and positive youth development.
In addition, this service will aim to provide overall improvement in the provision of services to young people in this district by providing:

  • Coordination of all service elements to ensure delivery of the TYHS

  • Leadership for youth health in the district, incorporating promotion and facilitation of workforce development and best practice approaches for other providers of youth health services.

Services are expected to contribute to reducing health inequalities and achieving Maori health objectives in particular, targeting services to impact on asthma, diabetes, injury prevention, smoking, hearing, mental health, oral health and immunisation, including violence prevention.

Whanau Ora Services
